Chronic Pelvic Pain

Chronic pelvic pain is an umbrella term for anything that causes prolonged, ongoing pain in the pelvic area. In particular, Chronic Pelvic Pain Syndrome should be taken seriously, as men who suffer from it often brush it off as a urinary tract infection or even a sexually transmitted disease due to the similar symptoms.

What Is Chronic Pelvic Pain Syndrome?

Chronic Pelvic Pain Syndrome is a long-standing prostate inflammation that is often misdiagnosed as its symptoms are often confused with urinary tract infections or sexually transmitted diseases.

Symptoms include:

• lower abdominal and groin discomfort
• the need to urinate often
• burning sensation when passing urine
• pain or discomfort with ejaculation

If your symptoms are prolonged and recurrent, there is a possibility that they are associated with Chronic Pelvic Pain Syndrome instead of an UTI or STD.

What Causes Chronic Pelvic Pain?

Muscles

The muscles that form the bowl of your pelvic floor can become very tight or in a knot, which lead to pain. People who exercise frequently may injure their pelvic floor or the nerves that supply to the pelvis. Those who masturbate excessively may also experience chronic pelvic pain.

Treatment For Chronic Pelvic Pain

Chronic Prostatitis

If the pain is caused by a chronic infection, a doctor or urologist will usually recommend oral antibiotics. Do note that you may need a long-term course of antibiotics, lasting up to 6 months.

If symptoms do not improve following oral antibiotics, other treatment options include:

• intravenous antibiotics for severe infections
• muscle relaxants to relieve spasm of the pelvic muscles
• non-steroidal anti-inflammatory drugs (NSAIDs) to relieve pain
• alpha blockers to treat urination difficulties
• surgery to remove scar tissue in the urethra, which can help urination difficulties
• therapy to help with psychological stress and anxiety

Muscles

If the pain is caused by a muscle problem such as pelvic floor tension, prolonged pelvic health physiotherapy or novel strategies like Extracorporeal Shock Wave Therapy (ESWT) are recommended.

ESWT is an emerging form of treatment for chronic pelvic pain. Low intensity shockwaves are delivered through a probe over your skin and directed towards your prostate glands or over muscles in your pelvis that are sore and contributing to the pain.

How do I know if I have Chronic Pelvic Pain?

Your doctor should first rule out any obvious urinary tract infections or sexually transmitted diseases that may explain your symptoms.

A rectal exam will then be performed to check the prostate for swelling or inflammation, while looking out for signs of infection, such as discharge from the penis or swollen lymph nodes.

What made the difference was that she was extremely patient and took the time to listen to me, and would sort out every single one of my concerns. She conducts her clinical sessions with extreme professionalism and I always feel comfortable around her. The best part was that she was never judgemental, and never made me incur any unnecessary costs. There are many doctors out there, but most do not take the time to really hear their patients out. I would highly recommend her if you are on the fence about seeing a doctor for your condition, because Dr Jess is a medical practitioner who truly cares about her patients' well being!

Dr. Jess is that excellent (and very rare) combination of big-hearted warmth and extreme clinical competency. I'd spent a while trying to find a general practitioner in Singapore with some very, very mixed results (although I will admit to being a difficult patient sometimes). Dr. Jess was recommended to me when I suffered an acute condition and I can't believe how lucky I am to have found her when I did. She has that fantastic ability to be both amazingly friendly and approachable whilst still being incredibly knowledgeable, professional and focused on getting the best outcome possible for your health needs at the same time. As someone who has a background history in medical research and teaching medical students, I'm constantly impressed with the way Dr. Jess has always used both trusted, conservative methods whilst also being up to date with new techniques and the latest knowledge, and will research for answers and present it in a manageable, down-to-earth and tailored way - or recommend excellent specialists when required. I cannot recommend her highly enough, and she would be my doctor of choice no matter where I lived in Singapore.
